[SM3] Minutes and updated outline posted from yesterday's SM meeting

I have posted the minutes from yesterday's Semantic Model meeting to:

	- Fix cover page of 5108.06 (says 5108.05)
	- Paul to post updated SM charter with new titles and abstracts
