James,
There are two issues here:
1) Only the sample file referenced from the spec is the one that is up to
date, the others are early versions and all had some problems.
2) I uploaded the wrong file -- my bad. Try the new "sample2.pdf" to see if
you get better results. (I removed the others to avoid any confusion in the
future.)
Let me know if you're still having problems with this file.
